Patient forms & registration

New patients

  • New patient registration (online)
  • New patient registration (paper)
  • We are currently accepting new patients at Minchinhampton Surgery. According to new NHS guidelines, we do not need to see a form of photographic identification or proof of address to register you as a new patient. Please check your home address is within the surgery’s catchment area before registering.
  • We recommend that all new patients book a 20-minute “new patient” appointment with a GP to run through their medical history and any existing medications. It is advisable that you make sure you have plenty of stock of any of your repeat medications from your previous surgery before registering here.

Carers

  • Carer registration form
  • Please let us know you are an informal (unpaid) carer so we can help you get the support & resources you need to care for someone. You can also find more useful resources on the “carers” section of our website.

Online services

  • Own online access form
  • Proxy online access form
  • We encourage all patients to get online access via the NHS App. You can use the app to easily book GP appointments, request repeat prescriptions, and view your medical record online.
  • Alternatively, you can request online access through the surgery via SystmOnline.
  • If you would like to apply for your own online access, please bring a completed online access form to reception alongside an original form of photographic identification (passport/driving licence).

Complaints

Update your contact details

  • Change of address form
  • Please inform us if you have recently changed your address, mobile or landline number, or email address so we can update your medical records.
  • You can also use your online services to update your contact details.

Out of area patients

  • Out of area application form
  • If your home address is outside the boundary of our catchment area, you are considered an “out of area” (OOA) patient. We recommend that all OOA patients register with a GP nearer to your home address.
  • OOA patients may request to register, or stay registered, at Minchinhampton Surgery – but it is at the discretion of the practice whether or not to accept OOA registered patients. This decision is made on an individual basis by the GP partners.
  • If you would like to apply as an OOA patient – please fill in an out of area application form. If you are new to the surgery, this form must be brought in with a new completed registration pack.

Temporary patients

  • We are able to see visitors to the area as “temporary patients”. Please call reception so we can complete the necessary temporary registration forms. There may be a charge for overseas visitors if there is no NHS agreement between their home country and the UK.
